Pursuant to Regulation 5 (1) of the Telecommunications (Universal Service) Regulations, whereby concessionaires are required to contribute annually such percentage of its gross annual revenue to the Universal Service Fund, the following Forms listed in Table 1 are to be completed by the relevant concessionaires, where applicable.
Forms must be submitted:
By regular mail or by hand, under confidential cover, to the Authority’s office at #5 Eight Avenue Extension, Barataria.
USF Gross Revenue Data Forms shall be submitted no later than six months after the end of the contributor’s financial year end.
