Market Data Forms
Pursuant to Section A28 of the Concession Agreement, the following Forms listed in Table 1 below are to be completed by concessionaires and submitted to the Authority via one of the following:
- Regular mail or by hand to the Authority’s Head Office, located at #5 Eighth Avenue Extension, Barataria
Market Data Forms to be submitted quarterly shall be submitted no later than six (6) weeks after the end of each calendar quarter. Market Data Forms to be submitted annually shall be submitted no later than six (6) weeks after the end of each calendar year. This requirement is necessary to facilitate the timely publishing of the Quarterly Market Brief and the Annual Market Report. A submission schedule is included in Table 2 below.

Determination 2010/01 Forms
Determination 2010/01 Forms
Pursuant to the Determination 2010/01, the following forms as listed in Table 3 are to be completed by concessionaires providing International Services and submitted to the Authority via one of the following:
- Regular mail or by hand to the Authority’s Head Office, located at #5 Eighth Avenue Extension, Barataria
As per Section 4.1 (4) of the Determination 2010/01, forms shall be submitted no later than fourteen (14) days after the end of the period to which the information relates. A submission schedule is included in Table 4 below.

2013 LRAIC Forms
The changes reflected in these forms relate to concessionaires operating cable TV and FWA networks.
For the CCA data request, the relevant cable TV concessionaires are requested to provide further details on the transmission infrastructure assets installed in their network, both in terms of types of assets and where these assets are installed.
The relevant concessionaires are further requested to provide information on the duct assets – in particular, the relevant share of ducts relating to the core and access network.
For the LRAIC data request, guidance to FWA operators and cable TV operators are included in the Annex. Accordingly, the transmission allocation and access allocation sheets are updated.
