INSTRUCTIONS TO PwD APPLICANTS

  1. The PwD Application Form must be completed, and all mandatory fields filled out.
  2. The PwD Application Form must be accompanied by the following supporting documents, if applicable:
    1. Copy of National photo identification as indicated on the PwD Application Form
    2. Copy of evidence of visual, hearing or other disability (medical report or doctor’s certificate)
    3. Copy of most recent Disability Grant or Invalidity Benefit payment (if paid by cheque)
  3. The “Terms and Conditions” of the subsidy must be agreed to and signed by the Applicant.  

Terms and Conditions

  1. The Applicant hereby consents to TATT’s use of their contact information to complete the processing of this application.
  2. The Applicant hereby consents to TATT’s use of any information regarding an Invalidity Benefit from the National Insurance Board of T&T (NIBTT) or Disability Grant from the Ministry of Social Development & Family Services (MSDFS). This information will be used to confirm the Applicant’s validity as a beneficiary and to authorize NIBTT/MSDFS to release such information to TATT.
  3. The Applicant understands that this Application does not guarantee their qualification for the subsidy forthe purchase of a mobile handset.
  4. The Applicant understands that this subsidy is a one-time offer and only valid for the purchase of one (1)mobile handset. 
  5. The Applicant understands that once approved for the subsidy for the purchase of a mobile handset, theyshall be solely responsible for the safety, security, and upkeep of the mobile handset.
  6. The Applicant agrees to adhere to all contractual obligations or terms and conditions entered into with itsmobile service provider.
  7. If for any reason, the subsidised mobile handset is stolen or otherwise removed from the Applicant’spossession, the Applicant shall inform the Authority of the theft or circumstances of its removal withinseven (7) days of the event. In the case of a stolen handset, the Applicant shall ensure that to the best oftheir ability a formal police report is made and inform the Authority promptly of this report for its records.
  8. The Applicant understands that the Authority is not responsible for any damage, defect or circumstancethat renders the subsidisied handset unusable.
  9. Where the Applicant is a minor, the signing party acknowledges these Terms and Conditions and shall beresponsible for ensuring they are understood and adhered to.
